“Widow Mourns Tragic Loss as Company Fined £400K”

A grieving widow expressed her enduring sorrow over the tragic death of her husband in a devastating accident involving a forklift. The deceased, Chris Keegan, lost his life while on duty for Hessle Plant Ltd at their main facility in Castleford, West Yorkshire.

Following the incident that claimed Mr. Keegan’s life on November 20, 2023, the company faced legal consequences. Leeds Magistrates’ Court imposed a substantial fine of over £400,000 on the employer, along with additional costs totaling £8,000 and a victim surcharge of £2,000.

The court proceedings revealed that the fatal accident occurred as Mr. Keegan was returning a forklift truck to a site in Sheffield after undergoing repair work. During the process of loading the vehicle onto a trailer early in the morning, the forklift slipped off the trailer, causing Mr. Keegan to be ejected from the seat and trapped between the forklift and another trailer.

Despite the efforts of emergency services at the scene, Mr. Keegan succumbed to his injuries, leaving behind his devastated wife, Dianne, who described him as a kind and generous man. She expressed her profound grief, stating that she will never come to terms with losing her husband in such a tragic manner.

An investigation by the Health and Safety Executive (HSE) uncovered critical lapses in safety procedures at Hessle Plant Ltd. The inspection revealed that the forklift involved in the accident had not undergone a comprehensive safety check prior to operation. Additionally, the examination post-accident identified several defects that should have been addressed beforehand.

It was also noted that the company failed to conduct full pre-delivery inspections on forklifts returned by existing customers, leading to operational risks. Moreover, a lack of seatbelt usage among employees operating forklift trucks was highlighted, with no effective monitoring system in place to enforce safety protocols.

Hessle Plant Ltd admitted to breaching Section 2(1) of the Health and Safety at Work etc. Act 1974. As a result, the company was fined £433,550 and directed to cover costs and a victim surcharge. HSE inspector David Beaton emphasized the preventable nature of the tragedy, emphasizing the importance of proper maintenance and safety measures for machinery like forklift trucks.

The inspector stressed the significance of maintaining equipment in optimal condition and enforcing seatbelt usage to prevent similar incidents in the future. The Mirror reached out to Hessle Plant Ltd for comment on the matter.
